My name is Kath Lancaster and I developed this model to improve outcomes for your children, young people and families.


By completing the questionnaire through the TLM Portal, you, your child or young person will have the opportunity to have a full health assessment and ask for help, support and information when it is needed.

The Lancaster Model

Built using a strong evidence base and Kath’s years of experience, the whole model has been developed to identify earlier any health and wellbeing needs.  Improved access with your local School Health team and other health services will help you to get the right support and information you need, when you need it.

How does it work?

Each web-based questionnaire will take about 20 minutes to complete and consists of research-based questions which together, assess and highlight health-related needs.

This process will be carried out by yourself (at school entry), by your child (in Year 6) and by your young person (in Year 9 and after they have had their 16th birthday).

What type of questions are asked?

Questions are asked about various important health-related topics, including Medical Problems, Emotional Health and Wellbeing, Lifestyle Choices and Lifestyle Behaviour.

  • Does your child have a long-standing illness?
  • Would you like help and support about child development?
  • How often does your child drink plain water?
  • In the past year have you been to the accident and emergency department following an accident or injury?
  • In the last year have you had a check-up at the dentist?
  • Have you ever been bullied?
  • Have you any worries and concerns about your diet?
  • In the past year have you been to the accident and emergency department following an accident or injury?
  • Would you like more information about emotional health?
  • How many people in your class do you think drink alcohol more than once a week?

Who sees the information and is it stored securely?

All identifiable information collected via the questionnaire will be seen first by the School health team and then shared appropriately with other professionals in the same way as any other health-related information.

We fully appreciate that you may want to know more about the information/data that is collected, and how it’s stored, and that it’s all safe. Aire Logic Ltd/The Lancaster Model complies with the recommendations for Data Security and Privacy that is required under current laws and legislation including GDPR, which was introduced across the EU and UK in May 2018.

Children and young people spend time online practically every day and it can be hard to keep track of your child’s activity on social media and apps. The NSPPC in conjunction with O2 have a campaign called Share Aware that offers practical advice and tips to help you.
