Re-allocation of caseloads directed by the skill mix strategy and the findings from the implementation of The Lancaster Health Needs Assessment
The provision of a knowledge and training development package which supports training, change management and mentorship
A process of evaluation provided one year after implementation of the chosen modules, establishing and measuring the effect of the service redesign
